Intervenciones no farmacológicas para reducir la depresión, sobrecarga y ansiedad en personas cuidadoras de mayores con demenciaun metaanálisis

Resumen

Objetivo: analizar la eficacia de intervenciones no farmacológicas orientadas a la disminución de la sobrecarga, ansiedad y depresión en personas cuidadoras de familiares con demencia. Metodología: revisión sistemática con metaanálisis de ensayos clínicos aleatorizados. Búsqueda en diversas bases de datos (PubMed, Cinahl, Scopus, Cochrane, PsychInfo, Lilacs, IME y Cuiden Plus) hasta febrero de 2017 (sin límites de tiempo ni idioma) y en la bibliografía de los artículos recuperados. Se han valorado intervenciones educativas, psicoeducativas y de terapia en relación con la depresión, la sobrecarga y la ansiedad. El análisis se llevó a cabo mediante un metaanálisis de efectos aleatorios con la d de Cohen como medida de efecto combinado. Los cálculos fueron realizados mediante el programa Comprehensive Meta Analysis versión 3.3.070. Resultados: se revisaron 613 estudios y se incluyeron 37. Se encontró asociación estadística negativa de las intervenciones psicoeducativas con la depresión, tanto en el corto (d= -0,55) como en el medio-largo plazo (d= -0,24), y con la sobrecarga en el medio-largo plazo (d= -0,17). Asimismo, se encontró asociación estadística negativa entre la terapia y la ansiedad y la depresión, tanto en el corto (d para depresión= -2,59 y d para ansiedad= -2,06) como en el medio-largo plazo (d para depresión= -1,40 y d para ansiedad= -2,34). Conclusiones: las intervenciones psicoeducativas tienen un efecto beneficioso sobre la disminución de la depresión y la sobrecarga en el medio-largo plazo. Las intervenciones de terapia tienen un efecto beneficioso en la disminución de la ansiedad y la depresión.
